RESUMEN Fundamento: el melanoma maligno es un tumor con una gran capacidad de invasión y de difícil tratamiento por su elevada agresividad y mortalidad, en este, los estudios morfométricos se hacen necesarios por el valor que aportan en el diagnóstico histológico para mejorar el pronóstico de la enfermedad. Objetivo: describir el comportamiento de indicadores morfométricos como el área nuclear, volumen nuclear y factor de forma nuclear. Métodos: se realizó un estudio de serie de casos de 12 pacientes con el diagnóstico de melanoma maligno de piel en el período septiembre 2015 a septiembre 2017 en la provincia Holguín. Se emplearon métodos teóricos y empíricos, estos últimos basados en técnicas morfométricas, luego se realizó análisis estadístico de los datos obtenidos y se reflejaron en tablas. Resultados: el área y el volumen nuclear muestran valores pequeños. El factor de forma como indicador de pleomorfismo nuclear presentó valores superiores en los casos del estudio que en los descritos en la literatura. Conclusiones: el volumen nuclear muestra valores pequeños, lo que habla a favor de un comportamiento característico de tejido hiperplásico, al igual que el comportamiento del área nuclear, los valores del factor de forma nuclear indican menor pleomorfismo que el descrito en la literatura.


ABSTRACT Background: malignant melanoma is an invasive tumour and difficult to treat due to its high level of mortality and aggressiveness, a morphometry study was required to provide a proper diagnosis of the disease. Objective: to describe the behavior of morphometric indicators as nuclear area, nuclear volume and nuclear shape factor. Methods: a case series study in 12 patients having malignant skin melanoma was carried out, in the period from September 2015 to September 2017 in Holguin Province. Theoretical and empirical methods were used; these last two methods were based on Morphometric techniques. Then a statistical analysis of the information obtained was performed and they were added on a chart. Results: the area and nuclear volume showed low rate. The shape factor showed less levels of pleomorfism than the ones described in specialized literature. Conclusions: when low values of nuclear volume and nuclear area in malignant melanoma are found we are dealing with hyperplastic tissue. The same happens with the incidence of the nuclear area; the values of the nuclear shape factor evidenced less pleomorfi levels than the ones described in specialized literature.

ABSTRACT

Abstract Choroidal melanomas are the most common primary intraocular malignant tumor in adults. They tend to be more malignant; because of their location hidden behind the iris they can not be detected until they become larger. Therapeutic strategy is related by size, extension, number and location of tumor and growth patterns. High frequency ultrasound biomicroscopy (UBM) gives high resolution, cross-sectional images of the anterior segment lesions. Postequatorial lesions and intracranial extension of the melanomas are scanned by magnetic resonance imaging (MRI). We report a case of bilobed tumor with confusing appereance in preoperative imaging studies and macroscopy following enucleation. MRI is the perfect imaging method to reveal extension and size of the tumor in the posterior chamber. Combined use of UBM and MRI provides appropriate staging of ocular melanomas.


Resumo Melanomas coroidais são os tumores malignos intra-oculares primários mais comuns em adultos. Eles tendem a ser mais malignos; devido à sua localização ser escondida por detrás da íris eles não podem ser detectados até se tornarem maiores. A estratégia terapêutica está relacionada com tamanho, extensão, número e localização dos padrões tumorais e de crescimento. O biomicroscopio ultra-sónico de alta frequência (BMU) fornece imagens transversais de alta resolução das lesões do segmento anterior. Lesões pós-equatoriais e de extensão intracraniana dos melanomas são digitalizadas em ressonância magnética (RM). Relatamos um caso de tumor com dois lóbulos, com aparência confusa em exames de imagem pré-operatórios e macroscopia após enucleação. A RM é o método de imagem perfeito para revelar a extensão e o tamanho do tumor na câmara posterior. O uso combinado de BMU e MRI fornece o faseamento apropriado dos melanomas oculares.

ABSTRACT Objective To analyze the distribution of larger diameter in the pathological report of cutaneous melanoma patients. Methods Data were obtained from patients seen from 1994 to 2015. Date, sex, age, maximum diameter, histological subtype, primary site, microscopic thickness, mitoses, ulceration, vertical growth phase, and regression were the variables studied. This study was approved by the National Ethics Committee - Brazil Platform. Patients were grouped into smaller diameter (≤6mm) and larger diameter (>6mm). The statistical analysis used the χ2test (p<0.05). Results Of the 292 patients analyzed, 123 were seen between 1994 and 2004, and 169 between 2005 and 2015; in that, 151 women and 141 men, mean age of 52 years. The diameters ranged from 2 to 76mm (mean of 14mm), 81 patients with smaller diameter (≤6mm) and 211 with larger diameter (>6mm). Out of 81 patients with smaller diameter, 29 had invasive melanoma, while 179 of the 211 with larger diameter were invasive. A difference was observed in frequency of vertical growth phase. Conclusion Pigmented skin lesions with diameter smaller than 6mm should not be an excluding factor for biopsies, especially when patients present risk of developing skin cancer.


RESUMO Objetivo Analisar a distribuição do maior diâmetro reportado no laudo histopatológico de portadores de melanoma cutâneo. Métodos Os dados foram obtidos de pacientes atendidos de 1994 a 2015. Data, sexo, idade, diâmetro máximo, subtipo histopatológico, sítio primário, espessura microscópica, mitoses, ulceração, fase de crescimento e regressão foram as variáveis estudadas. O estudo foi aprovado pela Comissão Nacional de Ética em Pesquisa na Plataforma Brasil. Os pacientes foram agrupados em diâmetro menor (≤6mm) e maior (>6mm). Análise estatística utilizou o teste χ2 (p<0,05). Resultados Dos 292 pacientes analisados, 123 foram atendidos entre 1994 e 2004, e 169 entre 2005 e 2015, sendo 151 mulheres e 141 homens, com média de idade de 52 anos. Os diâmetros variaram de 2 a 76mm (média de 14mm), sendo 81 pacientes com diâmetro menor que 6mm e 211 com diâmetro maior. Dos 81 pacientes com diâmetro menor, 29 apresentavam melanoma invasivo, enquanto 179 dos 211 com diâmetro maior eram invasivos. Houve também diferença de frequência da fase de crescimento vertical. Conclusão Diâmetro de lesões pigmentadas menor que 6mm não deve ser fator excludente para realização de biópsias, especialmente para paciente de risco para câncer de pele.

ABSTRACT

RESUMO O melanocitoma do disco óptico é um tumor benigno, com pouca predisposição a transformação maligna para melanoma. Geralmente é assintomático, contudo deve ser avaliado periodicamente com exames, sendo um deles a campimetria computadorizada que se encontra alterada em 90% dos casos. Relatamos o caso de um homem de 61 anos sem diagnóstico prévio de melanocitoma do disco óptico. Discute-se a importância de um diagnóstico diferencial com melanoma e acompanhamento seriado do paciente.


ABSTRACT The optic disc melanocytoma is a benign tumor, with little predisposition to become a melanoma. It is usually asymptomatic, nonetheless should be evaluated periodically, most often by computerized perimetry, which in 9 out of 10 cases will show some alteration. In the following paper we report the case of 61 years old man without previous diagnosis of optic disc melanocytoma and discuss the importance of melanoma differential diagnosis and monitoring.

ABSTRACT

The authors report fundus autofluorescence (FAF) and spectral-domain optical coherence tomography (OCT) findings of two consecutive patients who presented with optic disk melanocytoma (ODM). A retrospective study was performed by reviewing medical records and ophthalmic imaging examinations. Optical coherence tomography findings were sloped and brightly reflective anterior tumor surface, adjacent retinal desorganization and abrupt posterior optical shadowing. Vitreous seeds were found in one patient. Fundus autofluorescence revealed outstanding hypoautofluorescence at the tumor area and isoautofluorescence at the remaining retina. Optical coherence tomography findings of the reported cases are consistent with those reported in the reviewed literature. Fundus autofluorescence has been used in the assessment of choroidal melanocytic tumors, but not yet in melanocytomas. We assume that this is the first report of these findings and believe that when its pattern has become clearly defined, fundus autofluorescence will be a useful tool to avoid misdiagnosis in suspicious cases and for follow-up.


Os autores descrevem os achados do exame de autofluorescência do fundo de olho (AF) e da tomografia de coerência óptica (TCO) de domínio-espectral em dois pacientes consecutivos apresentando melanocitoma disco óptico (MDO) em um estudo retrospectivo, por revisão dos prontuários e exames oftalmológicos de imagem. Os achados no exame de tomografia de coerência óptica foram hiperrefletividade e elevação da superfície anterior da lesão, desorganização da retina adjacente, e, sombreamento óptico posterior. Sementes vítreas foram encontrados em um paciente. O exame de autofluorêscencia do fundo de olho revelou hipoautofluorescência marcante na área do tumor e isoautofluorescência nas demais áreas da retina. As características encontradas na tomografia de coerência óptica dos pacientes apresentados são consistentes com os achados relatados na literatura atual. A autofluorêscencia do fundo de olho tem sido utilizada na avaliação de tumores melanocíticos da coroide, mas ainda não em melanocitomas. Acreditamos que este seja a primeira descrição dos achados de autofluorêscencia do fundo de olho em pacientes com melanocitoma de disco óptico e que, quando seu padrão de tornar-se claramente definido, a autofluorêscencia do fundo de olho será uma ferramenta útil para evitar erros de diagnóstico em casos suspeitos.

ABSTRACT

El enfrentamiento a una lesión pigmentada cutánea es frecuente en la práctica médica diaria. Dado que el diagnóstico diferencial incluye al Melanoma Maligno (MM) y considerando que su pronóstico está claramente determinado por la precocidad del diagnóstico, es primordial conocer sus formas de presentación clínica. Además, es muy relevante conocer aquellas lesiones cutáneas que pueden simular clínicamente un MM. El objetivo de este trabajo es describir las características morfológicas del MM y entregar una descripción breve de sus simuladores.


Pigmented skin lesions are common in the daily practice of a physician. Since the differential diagnosis includes Malignant Melanoma (MM) and considering that its prognosis is determined by the precocity of the diagnosis, is essential to know its different clinical features. In addition, its critical to know how to identify MM simulators. Theaim of this work, is to briefly describe the clinical spectrum of MM and its most frequent simulators.

ABSTRACT

Objective To detect the level of serum melanoma-inbibiting activity (MIA) in patients with uveal melanomas, and investigate the value of MIA in diagnosing and inspecting uveal melanomas. Methods Enzyme-linked immunosorbent assay (ELISA) was used to detect the concentrations of MIA in peripheral serum of 27 patients with uveal melanoma, 6 with melanocyte tumor, 7 with other ocular tumors and 16 healthy individuals, respectively. Results The concentration of MIA in patients with uveal melanoma was significantly higher than that in the healthy ones (16 individuals) and the patients with adenoma of non pigmented ciliary epithelium (4 patients), retinoblastoma (2 patients), and retinal angioma 91 patient). The concnetration of MIA in patients with uveal melanoma without scleral infiltration or remote metastasis was obviously lower than that in the patients with scleral infiltration or remote metastasis, but didn′t differ much from which in the patients with melanocyte tumor. In the patients with uveal melanoma without infiltration or remote metastasis, there was no significant difference of MIA level between patients with spindle cell and mixed and epithelioid cell. Conclusion The level of serum MIA may be an effective index in diagnosing uveal melanoma, which can monitor the metastasis of uveal melanoma.
